Immutable versions

Every successful publish creates a ready immutable revision. Activation is a compare-and-swap pointer move. Pass the current active version, or explicit null for the first activation. A race keeps the new revision ready and inactive. Activating or rolling back code never rewinds KV values, files, members, or network policy.
